RARC M44: Missing/invalid condition code

This is a Remittance Advice Remark Code (RARC, M-series) — a remark that supplements a CARC on the remittance advice (ERA/EOB) to add detail. Codes appear on the EOB / ERA after a claim is adjudicated.
